Salary Range: $65,000-$67,340
I. JOB SUMMARY
Reporting to the Program Director and Nurse Practitioner, the Licensed Practical Nurse (LPN) ensures the medical well-being of residents including the assessment of patient health problems and needs, the development and implementation of nursing care plans, and the maintenance of medical records.
II. ESSENTIAL FUNCTIONS / RESPONSIBILITIES
A. Obtains and completes patient histories and medical information on admission. This includes taking and recording vital signs and measurements, and collecting specimens as required.
B. Provides prescribed physical care and carries out therapeutic and medical regimens. Administers prescribed medications to UC’s as directed by Physicians.
C. Educates and advises patients, as indicated, on health related issues such as sexuality, substance abuse, nutrition, personal hygiene, exercise and medication per Agency and ORR mandates and per individual patient needs and care plans.
D. Observes residents for changes in physical condition, appearance and behavior; reports possible abnormal symptoms to the Physician or Clinician.
E. Responds to medical emergencies on campus. Provides first aid to residents and workers as required and/or refers patients for follow up medical care at hospital emergency room or private physician.
F. Will schedule and coordinate medical appointments. Will also assist with transport of patients to and from all necessary medical appointments in the event the patient’s medical complexity warrants a nurse’s presence to ensure adequate provision of care.
G. Prepares treatment room and assists in stocking and maintaining medical supplies and equipment.
H. Maintains accurate and up to date patient records (via paper chart and electronic), including treatments and the administration of prescribed medications.
I. Effectively build relationships and ensure that information is gathered or shared with the appropriate staff and stakeholders. Participate in case conferences and staff meetings to ensure coordination of services.
J. Assist the ORR team with the development and implementation of a health education program for youth and health-related workshops for facility staff.
K. Conduct and assist with regular quality assurance activities to ensure that UCs are safe and comfortable, and receiving appropriate care.
L. For the protection of all youth and staff ensure the immediate reporting of any knowledge or suspicion of sexual abuse, sexual harassment, inappropriate sexual behavior, and staff code of conduct violations. This may include timely submission of a SA/SIR, assessment of injuries, notification to appropriate providers, and/or assistance with investigations.
M. Performs other related administrative and direct care duties as required.
III. JOB REQUIREMENTS
A. Knowledge:
• Maintain current information and knowledge of ORR policies and procedures to ensure that the program operates effectively and efficiently in accordance with all regulations and guidance.
• Knowledge of relevant Nursing protocols (i.e., dispensing of medications), Agency health care policies, NYS Department of Health and Center for Disease Control mandates.
• Knowledge of basic first aid principles and techniques.
• Knowledge of cardiopulmonary resuscitation (CPR) principles and techniques.
B. Physical Skills and Abilities:
• Ability to sit and use a personal computer for extended periods.
• Ability to access all program locations and to climb stairs.
• Must be able to operate a motor vehicle.
• Must be able to take and record vital signs and measurements and collect specimens.
C. Mental Skills and Abilities:
• Bilingual in Spanish and have excellent bilingual oral and written communication skills.
• Ability to establish and maintain positive rapport and effective working relationships with the youth, sponsors and the ORR team of providers and administrators.
• Ability to effectively communicate with staff and medical providers in the community.
• Ability to develop and maintain required written program documentation in accordance with regulatory guidelines.
• Ability to accurately calculate and dispense prescribed doses of medications.
• Ability to effectively manage crisis situations involving emotional and/or behavioral needs of patients.
D. Working Environment:
• General office setting.
• Clinical environment with exposure to infectious diseases.
• Exposure to youth with trauma histories and concurrent physical / mental health disorders.
E. Education / Experience:
• You possess a bachelor’s degree.
• You have a minimum of two years’ experience as an LPN.
• Experience with the treatment of children and/or adolescents in a clinic or Health Services Department preferred.
F. Licenses/Certifications:
• You possess a bachelor’s degree.
• You have a current NYS License and Registration as a Practical Nurse.
• You have a minimum of two years’ experience as an LPN.
• You possess or will be able to obtain Therapeutic Crisis Intervention certification.
• You possess a valid NYS Driver’s License.
